As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Darwin. Some of your duties will involve processing referrals aiding clients in commencing with our services and continuously adjusting care plans to suit evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.
Additionally youll contribute to program coordination by assessing improving and overseeing operational and administrative systems. You will deliver exceptional service to ensure our clients and their families feel valued and supported while also fostering the growth of our aged care services.
This is a Permanent Full Time opportunity based in our Darwin office with some travel required to visit clients in their homes.
