Description:

Saint Francis Hospital and Medical Center and a member of Trinity Health Of New England is looking for a Crisis Clinician to join our team.

The Crisis Clinician / Social Worker / Therapist provides psycho-social services to adult adolescent and child patients seen in the Emergency Department Saint Francis Hospital & Medical Center. The Clinician conducts a comprehensive assessment to identify psychosocial concerns which interfere with recovery from illness or prevent a healthy lifestyle. Provides consultation to other health care providers to assist in the management of psychosocial problems interfering with compliance with other treatment regimens.

You Will Be Responsible For:

  • Performs assessments provides support to patients and families and is a patient advocate to assure appropriate cares is received.
  • Evaluates patient safety and collaborates with the healthcare team to makes appropriate referrals for treatment.
  • Effectively communicates with patients families and the integrated clinical team to ensure everyone understands the needs of the patient to ensure a safe discharge plan is created.
  • Demonstrates problem solving conflict resolution and positive communication through teamwork.

Requirements:

  • Education: MSW degree from an accredited School of Social Work or Behavioral Health Field required.
  • CT licensure in appropriate professional designation preferred i.e. Clinical Social Worker Psychologist Marriage and Family Therapist or Professional Counselor.
  • Excellent written and verbal communication knowledge of community resources social work theory and practice.
