About the Role

You will work as part of the SVMH Complex Care Program provides a specialist service for consumers experiencing high levels of complexity by working collaboratively with the consumer treating team services and carers. You will hold a housing and homelessness portfolio which will enable you to provide consultation and coordination support to clinicians working with a complex mental health client group.

  • Work within the Strengths Model of Recovery orientated practice. Actively uphold a culture of hope by using optimistic language supporting people and celebrating consumers recovery efforts.
  • Scoping and analysis of housing needs and issues for SVMH consumers.
  • Develop and engage in strategic planning and development to help SVMH to identify and take action to address housing issues for SVMH consumers.

Classifications for this position will range from

  • RPN Grade 4 NP75 - NP77 ($107577.60 - $115850.80 per annum pro rata)
  • OT/SW Grade 3 YB24 - YB27 or YC46 - YC49 ($97650.80 - $111566 per annum pro rata
  • Psychologist Grade 3 PL1 PL4 ($108279 - $123905 per annum pro rata)


Your Contribution

  • A minimum of at 5 years post qualification clinical experience within Mental Health for SW OT Psychology and RPN.
  • Expertise / Extensive knowledge of the Housing and Homelessness sector.
  • Registration with Australian Health Practitioner Regulation Agency (all disciplines except Social Work).
  • RPNs - Nursing and Midwifery Board of Australia (NMBA). Relevant postgraduate qualification in mental health.
  • Occupational Therapists - Registration with the Occupational Therapy Board of Australia.
  • Psychologists - Registration as a psychologist under the Health Practitioner Regulation National Law Act (2009). Practice endorsement as a Clinical Psychologist or eligibility to enter a Clinical Psychology registrar program with the Psychology Board of Australia; or
  • Social Workers - An approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers
  • Current Victorian drivers license
  • A valid Nationally Coordinated Criminal History Check or willing to obtain

About Complex Care
The Complex care team provide secondary consultation coordination and brokerage services to the St Vincents Mental Health Program. Providing specialist advice and assistance to community case managers and in-patient clinicians is a key function of the service.

LOCAL WORK ENVIRONMENT

St. Vincents Mental Health (SVMH) provides high quality recovery-oriented services across an adult area mental health service for the inner city areas of Yarra and Boroondara. The service utilises the Strengths Model of recovery and this model is embedded across all parts of the mental health service. St Vincents is also committed to the development of a strong peer support workforce. The adult mental health service components are: The Footbridge Community Care Unit (CCU) the 44 bed Acute Inpatient Service (AIS) located on the St. Vincents Hospital site the North Fitzroy PARC and two community mental health services (CMHS) at Hawthorn (Hawthorn CMHS) and East Melbourne (Clarendon CMHS). Community mental health services comprise the Triage and Crisis assessment and Treatment (CAT) service Continuing Care (CCT) Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services. The Primary Intervention and Care Team (PICT) includes Primary Mental Health Extended Triage and HOPE (Suicide Prevention). A Consultation & Liaison Service is provided to the health service. NEXUS Dual Diagnosis Service the Body Image and Eating Disorders Assessment and Treatment Service (BETRS) and NEVIL Training Cluster are regional services. State-wide services are the Victorian Dual Disability Service and the Victorian Transcultural Mental Health. The Acute Inpatient Service includes 5 beds designated as the Koori State-wide Inpatient Service and linked to the Victorian Aboriginal Health Service.

St Vincents Mental Health is committed the recruitment and retention of high quality staff. The service aims to support staff to maintain and develop their skills to provide excellent clinical care and to meet their own personal and career goals. SVMH has a strong education and research focus in addition to affiliations with a number of universities and educational institutions. All SVMH clinical staff are expected to be familiar and practice in a manner consistent with the National Practice Standards for the Mental Health Workforce.

POSITION PURPOSE

The SVMH Complex Care Program provides a specialist service for consumers experiencing high levels of complexity by working collaboratively with the consumer treating team services and conjunction with severe mental illness these complexities can include recurring homelessness co-existing substance use issues cognitive difficulties challenging behaviour or patterns of offending behaviours and associated interaction with the criminal justice system comorbidities and high users of services.

The Complex Care Program provides Coordination Consultation and Brokerage support to the SVMH Adult Mental Health Program. Coordination focuses on complex discharge planning from bed based services as well as supporting care teams experiencing high level of complexities in the community. Secondary Consultation to discuss complex cases is provided to clinicians of the service and specialist NDIS consultation is provided by the NDIS Senior Clinician. Brokerage is also available to support consumers to reduce risk of hospitalisation and/or risk of disruption of community tenure.

The role of the Senior Clinician is to build a strong housing and homelessness portfolio working in collaboration with consumers and their families/carers clinicians and treating teams in identifying housing pathways to support access to appropriate stable accommodation. A focus of the role is to identify and address issues relating to housing homelessness and inappropriate housing for SVMH consumers and to also work closely with housing providers and other relevant services (i.e. DFFH Homes Victoria Housing Services NDIS providers etc) via liaison advocacy networking and strategy.

The role will include initial scoping identifying and consulting with key stakeholders and also providing consultation as part of the Complex Care Program. Supporting SVMH case managers and clinicians consumers and carers in relation to housing information and options is key to the role. Sharing and supporting clinicians in increasing their skills and knowledge in addressing housing issues with their consumers is also an integral part of the role.

This program is under the guidance of the Manager Footbridge Community Care Units PARC and Complex Care who also is the SECU Liaison Officer.
