Hospitals Coordinator - 4615

RESPONSIBILITIES

• Supervise team of 5 staff (2 Case Managers/3 Outreach Workers) and coordinate team activities focused but not limited to homeless individuals engaged at Lincoln Hospital.
• Maintain familiarity with medical and social services and resources available both on-site and in the community
• Provide referrals care coordination and direct assistance to support clients applying and/or attending appointments for: Medicaid/Medicare mental health services medical care and substance treatment
• Coordinate and lead case conferences with hospitals/medical providers and social service providers
• Conduct comprehensive needs assessments and psychosocial reports with clients
• Document all interactions with or on behalf of clients
• Provide direct counseling services as assigned
• Attend regular supervision sessions with direct supervisor
• Conduct field visits to engage and build rapport with chronically homeless individuals as required
• Drive agency vehicle to and from client locations and appointments
• Complete regular program reports
• Perform additional duties as assigned

QUALIFICATIONS

• Masters degree in social work public health or a related field. LMSW preferred
• Candidate with a valid drivers license preferred but not required
• Experience within a hospital and/or residential setting where medical psychiatric and medication monitoring services are provided is preferred
• Ability to engage and build rapport with populations with high support needs
• Strong oral and written communication skills
• Detail-oriented and resourceful with strong time management and organizational skills
• Proficiency in Microsoft Office suite and other standard business technology
• Bilingual/multilingual preferred but not required

PHYSICAL REQUIREMENTS

• Ability to use a computer for prolonged periods.
• Ability to occasionally lift and/or move up to 10 pounds.
• Ability to stand walk or sit for long periods of time.
• Ability to climb five flights of stairs if required to conduct home visits and/or fieldwork.
• Ability to bend and retrieve objects and/or documents.
• Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
• Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather including inclement weather if required to conduct home visits and/or field work.
• TB Test required with the first 120 days of employment.
