Position Overview:
Under the supervision of the Outreach Program Supervisor the Outreach Team Lead position will be responsible for developing and maintaining partnerships and creating/organizing outreach activities to educate promote and connect community members to all HCPH programs/services specifically on behalf of ACCESS and Healthy Start. The Outreach Team Lead is responsible for overseeing and supervising a team of Community Health Workers and providing guidance for their day-to-day tasks and professional growth.
Outreach Team Leads are direct liaisons for HCPH programs in the development implementation and updating of outreach strategies created to support all HCPH programs.
Duties & Responsibilities:- Oversee manage and train a team of Community Health Workers and provide day-to-day guidance.
- Provide leadership to support the outreach team in coordination of program strategies objectives outcomes and trainings.
- Utilize a data-driven approach to research and find partners/locations in vulnerable or high-risk areas to deploy personnel for the distribution of information resources and awareness of HCPH programs throughout Harris County.
- Track county-wide statistics such as Social Vulnerability Index and indicators specific to programs with the goal of mitigating disparities in priority populations and promoting HCPH programs.
- Lead strategy development implementation data collection reporting and evaluation of outreach efforts for HCPH program(s) to demonstrate impact of increased utilization of HCPH programs.
- Create and provide trainings/workshops/presentations for staff or community members/partner organizations.
- Actively participate in outreach activities that strengthen partnerships and educate the community on HCPH programs/services external resources and relevant public health topics.

Requirements
Education: High school Diploma.
Knowledge Skills and Abilities: Intermediate proficiency in Microsoft Office. Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment. Ability to maintain confidentiality when dealing with sensitive information. Develops the ability of others to perform and contribute to the organization by providing ongoing feedback and providing opportunities to learn through formal and informal methods. Must use your personal mobile device for our Multifactor Authentication process.

Position Type and Typical Hours of Work: This is a full-time position. Days and hours of work are typically Monday through Friday from 8am to 5 pm. Other days and hours as necessary to meet needs of team.
Work Environment: This job operates in a professional office environment. This role routinely uses standard office equipment such as computers phones photocopiers filing cabinets and fax machine.
Physical Demands: This is a largely stationary role however does require field work and many duties would require the ability to lift up to 20 pounds and bending and standing for periods at a time. This position requires driving of county vehicles on a week-to-week basis.

Employment may be contingent on passing a drug screen and meeting other standards.
