Under the supervision of the Infectious Disease Program Manager and as a member of the multidisciplinary team the Infectious Disease (ID) Program Coordinator is responsible for Fenway Healths ID Program operation implementation including: administrative and clinical tasks associated with daily performance of the HIV treatment and prevention programs Long-Acting Injectable (LAI) program Hepatitis C (HCV) program and direct patient care coordination.
Representative Duties:
Coordinates and facilitates clinical care for patients with ID appointments as well as ongoing patient support following their appointments.
Performs administrative tasks pertinent to programmatic day-to-day operations
Effectively communicates with patients co-workers on multidisciplinary team in medical department and across departments and with external agencies
Policies and Procedures
Participates in quality improvement initiatives
Meets Agency Participatory Expectations
Performs related duties as required
