Job Description

We are seeking a Social Worker with hospital experience and EPIC experience for a position involving discharge planning and comprehensive psycho-social assessments in the healthcare setting. Responsibilities include conducting assessments collaborating with the interdisciplinary team to develop care plans facilitating efficient patient transitioning and advocating for patients and families with health care providers and insurance companies. The ideal candidate will have strong communication skills be familiar with the Rochester area and be able to interpret regulatory requirements to optimize patient placement. A Bachelors or Masters degree in Social Work is required for this role. Social Worker (Allied)
