Adult Protective Service Case Worker

Job Scope: Adult Protective Service (APS) case workers provide a full range of investigative care management and follow up services to alleged victims of abuse neglect or exploitation who are over 60 or persons with disabilities between the ages of 18 and 59. APS case workers have a thorough understanding of aging and disability services and resources. Case workers participate as an integral member of a multi-disciplinary community based social services team
Primary Responsibilities
  • Conduct and complete APS investigations substantiations case work and follow-up services
  • Provide care planning and service implementation
  • Coordinate client care with internal and external service providers
  • Receive and respond to intakes referrals and crisis situations
  • Educate clients and their families on choices/options for care benefits and available community resources
  • Monitor care and advocate for the client to ensure needed services are provided in a timely efficient and effective manner.
  • Assess changes in the clients status and implement services to address areas of need
  • Effectively use available internal and external consultation and resources including legal behavioral health IDOA and Age Options as needed to provide effective care
  • Participate in program planning and evaluation as an active cooperative member of the Older Adult Team
  • Participate in supervision as directed and in regular performance evaluations
  • Work cooperatively with other Older Adult Team and agency units/staff to maintain smooth continuity of care when cases are shared
  • Complete documentation in an accurate and timely manner to support program compliance and billing.
  • Complete and maintain IDOAs APS and CCC/CCP certifications including APS Supervisor certification as directed by APS program coordinator
  • Participate as a member of APS Multidisciplinary Team
  • Participate in internal and external staff development seminars and training programs
  • Interpret and communicate at the service delivery level agency policies and procedures
  • Ensure client confidentiality in accordance with KYC policy and HIPPA requirements
  • Responsible for all APS billing and compliance

Education and Experience:

  • Bachelors or Masters Degree from an accredited college or university in gerontology social work psychology or related discipline

  • A minimum of 2 years experience providing social services to older persons and/or persons with disabilities
  • Maintain all required IDOA Certifications including APS and CCP
Schedule/Time Keeping/Time-Off
  • Exempt Full-Time Hourly 37.5 hour work week
  • On Call Coverage is Required
