Job Description
We seek candidates with a Masters degree in Social Work for the role of Social Worker MSW Case Management. The person in this role will provide comprehensive services to include psycho-social assessments community resource information and referrals to patients their families and loved ones ranging in age from infants through geriatrics as it supports the continuum of care. She/he will provide coordination of complex discharge planning needs and facilitate multidisciplinary care team involvement to expedite plan of care for patients.
Candidates must have a Masters Degree in Social Work (MSW) from an accredited school of Social Work or Masters level degree in related field with equivalent experience. Must have one (1) or more years of experience preferably in an acute care hospital setting. Must have a wide variety of experience in provision of Social Work services. Strong preference given to candidates with a Clinical Social Worker License (LCSW) or who would be license eligible upon hire.
