Job Description

ON-CALL POSITION
Schedule: Monday Friday 9:00 AM - 5:00 PM
On call schedule: Every other week (evenings and weekends)

Eligibility Requirements

  • You must have 1 year of experience working directly with individuals with intellectual or developmental disabilities.
  • You must have a bachelors degree in a human services field such as social work psychology sociology special education or rehabilitation counseling.
  • You must have a valid drivers license & a clean driving historyto drive company vehicles
  • You must have reliable transportation

Pay

  • Base rate: $27/hour

  • This is a nonexempt position & is eligible for overtime

About the Position

The QIDP is a psychologist social worker or human services professional with specific higher education accomplishments and experiences that enable them to ensure that the services a client receives are consistent with their needs & wishes. You areresponsible for integrating coordinating and monitoring each clients progress towards the goals set in their Individual Support ensure Rimlands legal compliance with theState of Illinois Department of Human Services and are knowledgeable aboutDHS Rules 116. Some of your responsibilities include:

Client Advocacy

  • Participate in all Personal Care Plan meetings to aide in the development of the Implementation Strategy and calendar of life
  • Assist with bi weekly medical clinic
  • Communicate issues related to the safety and well-being of clients to supervisors
  • Maintain good relationships with physicians volunteers and similar community organizations
  • Ensure needed medical supplies are available to carry out identified outcomes per the individuals plan andorder as needed
  • Assist with the monthly medication changeover

Documentation

  • Complete a monthly summary of medical appointments physician recommendations monthly weight medication changes and any other medical specific changes that month.
  • Assure medical appointment summaries are completed following each appointment complete with the individuals response to the appointments physicians concerns and recommendations. Physician report to be attached in Therap.
  • Schedule follow up appointments as needed

Training

  • Monitor and review client skill training specific to medical and hygiene through assessments and direct observations
  • Provide On the Job Training to new hires through shadowing doctor appointments
  • Provide training reinforcements for areas such as: HCBS Rule 116 115 & 119 and personal plans
  • Train staff on the individuals Implementation Strategy and medical specific outcomes

Compliance

  • Ensure all medical appointments are completed consistent with DHS Rule 116 requirements
  • Ensure all medical needs are met consistent with physician recommendations and applicable staff training is conducted by the Health & Wellness Department.

Other duties and projects as assigned.

Location & Schedule

Note: You must be able to work the entire shift every week

Requirements

You must meet the following requirements:

Pass:

  • A fingerprint criminal background check

  • A child abuse and neglect check

  • A drug screen.

Provide:

  • You must have a bachelors degree in a human services field such as social work psychology sociology special education or rehabilitation counseling.

  • Residency for the past 2 years

  • Current & original documents that verify your identity & ability to legally work in the United States

  • Two verifiable employment references within the last 3 years
