Job Description

Licensed Home Care Physical Therapist Assistant (PTA) -Enjoy independence and flexibility to work in the home health field to care for patients in their home environment.

Hours: Full-time Monday - Friday 8:00am - 5:00pm

Pay and Benefits:

  • Hourly Pay Range: $31.40 - $37.44

Territory:This position will work in the field helping patients in their home setting. The office is inRoseville MN and the general territory of where the patients live for this position will be the northeast metro.

A few must-haves for this position:

  • Currently licensedas a Physical Therapist Assistant in the State of Minnesota. License is active and in good standing.
  • Two (2) years of experience as a PTApreferred.
  • Able to travel within geographic area serviced by the driverslicense reliable transportation and proof of auto insurance.
  • CPR Certification

Here are some functions of what you would do as a Licensed Physical Therapist Assistant:

  • Participate in the development and evaluation of the plan of care.
  • Assists the treating Physical Therapist in documenting a patients progress and response to treatment.
  • Completes and submits timely legible accurate relevant and sufficient documentation addressing the treatment and interventions provided the patients response to the treatment and interventions and the patients progress toward goals in accordance with professional standards of practice policy and procedures and payer requirements.
  • Provides services that are ordered by the physician as indicated in the plan of care.
  • Counsels and educates the patient and the family and other caregivers to promote patient progress toward mutually established goals and in meeting healthcare related needs.
