What you bring to SBHG:
- (4) years of experience in a social service field OR
- Associate Degree plus Two (2) years of experience in a social service field OR
- Bachelors degree in a social science field plus at least Six (6) months experience in a social service field or a school internship in a social service field
- Valid California Drivers License.
- Bilingual in Spanish preferred
How you will make a difference:
- Provides services at the TAY Center in the community and in TAYs homes.
- Uses personal vehicle to drive within the community to provide services and to transport TAY and/or TAYs family members in personal vehicle.
- Provides case management services to clients and their families as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team.
- Participates in meetings such as Treatment Team Meetings to ensure clients needs are met and recommendations are followed through.
- Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established by Stars.
- Assists the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.
