Job Description
A healthcare facility west of Rochester NH is seeking a compassionate Director of Case Management to join their team! Details are listed below.
Pay Range: $105000 - $120000 / Annually
*Pay based on experience and qualifications
Director of Case Management Opportunity:
- Full-time direct-hire opportunity
- Monday - Friday Day Shift
- Occasional On-Call required
- Possibility of quarterly bonuses
Director of Case Management Responsibilities:
- Director of Case Management assumes responsibility for the day-to-day operations and human resource management of the Case Management department
- The Director of Case Management has 5 Direct Reports underneath them
- Provides guidance and support to Case Managers and other staff including training on managing caseloads and interpreting regulations policies operational procedures and objectives
Director of Case Management Qualifications:
- Must be either a Licensed Registered Nurse Licensed Social Worker or Therapist
- 3 years of management experience
- 3 years of hospital-based Case Management experience including Utilization Review and Discharge Planning experience
- CCM or ACMTM certification is preferred
