Under the general supervision of the Program Director and/or Associate Director -The Case Manager supports individuals with serious mental illness and co-occurring disorders who reside in Treatment Apartment Program (TAP) housing. This role promotes recovery community integration and independent living through individualized care planning supportive skill-building and coordination of services. The Case Manager uses a social integration approach helping residents build meaningful relationships access community resources and participate in social educational and vocational opportunities that support long-term counseling wellness.
ESSENTIAL JOB FUNCTIONS:
Home Visits & Client Engagement
- Conduct routine home visits to assess client well-being housing stability and support needs.
- Build collaborative trusting relationships with residents that foster engagement in services and community life.
Service Planning & Recovery-Oriented Care
- Complete intake assessments and develop individualized service plans (ISPs) that reflect client goals and strengths.
- Incorporate goals that promote social inclusion connection and participation in meaningful roles and activities.
- Update ISPs regularly and document progress in alignment with program and regulatory guidelines.
Supportive Counseling & Skills Training
- Provide counseling focused on mental health wellness and social-emotional development.
- Offer coaching and psychoeducation on daily living interpersonal skills and community navigation.
- Use cognitive-behavioral and motivational techniques to promote behavioral change and skill acquisition.
Social Integration & Community Participation
- Support clients in identifying and engaging in social recreational educational spiritual and volunteer opportunities.
- Encourage the development of natural support networks peer relationships and reduced reliance on formal systems.
- Facilitate or co-lead groups that promote social connection identity exploration and recovery narratives.
Care Coordination & Advocacy
- Coordinate behavioral health primary care substance use treatment vocational and entitlements services.
- Assist clients in accessing benefits securing documentation and navigating public systems.
- Advocate for client needs within ICL and with external partners to promote equity and inclusion.
Crisis Support & Risk Management
- Provide crisis intervention and stabilization in the community or home settings.
- Participate in safety planning and reporting of incidents per agency protocols.
- Use trauma-informed approaches when responding to acute needs or conflict.
Housing Retention & Apartment Oversight
- Promote independent living through coaching on lease compliance conflict resolution budgeting and household care.
- Conduct periodic apartment inspections to ensure safety and habitability.
Team Collaboration & Communication
- Participate in team meetings supervision and interdisciplinary case conferences.
- Maintain open professional communication with all staff and support coordination of care.
Compliance & Documentation
- Maintain timely and accurate case notes assessments progress notes and required reports.
- Ensure documentation aligns with internal funder and regulatory standards.

ESSENTIAL KNOWLEDGE SKILLS AND ABILITIES:
Required Skills and Abilities:
- Commitment to person-centered recovery-oriented and trauma-informed care.
- Understanding of social integration principles and the role of community connection in behavioral health recovery.
- Ability to develop individualized service plans that incorporate housing clinical and social integration goals.
- Strong organizational and communication skills; ability to manage field-based work.
- Proficiency in Microsoft Office EHR platforms and documentation standards.
- Ability to work collaboratively with diverse populations including culturally responsive engagement.
Work Environment:
- Community-based position requiring regular travel to client apartments and partner sites.
- May require flexible hours including evenings and participation in on-call rotation
QUALIFICATIONS AND EXPERIENCE:
Bachelors degree in social work psychology rehabilitation or a related human services field and at least 1 year of direct experience working with individuals with behavioral health needs.
Associates degree in a related field with 2 years of relevant experience may be considered.
Candidates with less experience may be hired as Case Manager Trainees.
