Job Description
Overview
Dignity Healths Pacific Central Coast Health Centers (PHC) is a non-profit Community clinic organization comprised of more than 40 health centers located primarily from Ventura to Templeton spanning the coast. Our experienced physicians and advanced practice providers offer a wide range of primary and specialty care services. PHC is a part of Dignity Health Central Coasts award-winning network of outstanding hospitals imaging centers laboratories and post-acute services.
Responsibilities
In keeping with the Pacific Central Coast Health Centers health care philosophy the Care Team Access APP is credentialed with advanced training in areas of primary health care for adult and pediatric patients including but not limited to: taking health history performing physical examination and psychosocial assessments management of acute and chronic medical conditions. Provides clinical administrative services supporting Physicians in the clinic location
Qualifications
Minimum of 5 years experience as a PA
PA State Certification.
Advanced training in physical assessment and pharmacology; continuing education in field of practice
