Job Description

Initiates and coordinates multidisciplinary discharge planning activities beginning on day of admission for all in-patients skilled and extended care patients/residents. Assesses psychosocial needs of patients/residents. Formulates modifies and monitors multidisciplinary compliance to planned discharge activities. Assists patients and families in identifying and accessing community based resources. Coordinates referrals to external community resources and medical services as identified. Coordinates all resources and educational activities as needed. In addition this position will assist patients in the Emergency Department Rural Health Clinic and other areas of the hospital with psychosocial and discharge/placement needs. May occasionally be utilized on the med-surg floor to assist with patient care.

Depending upon the environment of care the following age groups are considered: infancy (birth to 12 months) toddler (1-4 years) preschool (4-6 years) school age (6-12 years) adolescents (12-18 years) early adult (18-44 years) middle adult (44-59 years) late adult (60 years and older).

Key Accountability

  • Completes initial psychosocial assessments and provides direct services to patients/residents and their support systems based on referral and complexity of need.
  • Evaluates health social and family dynamics counsels and engages the individual/support system in developing system in developing interventions and/or treatment plan.
  • Provides assistance and advocacy follow-up in obtaining financial resources i.e. food stamps VA Medicare Medicaid transportation placement etc.
  • Provides referral and or/coordination regarding emergency services homeless shelters housing food and clothing etc.
  • Evaluates community education needs on an annual basis. Plans and/or coordinates ongoing community educational programs within CCH and in the community on identified topics/health programs of interest.
  • Provides adequate references/resources/written educational information for patients/families and community.
  • Responsible for initiation daily coordination and monitoring of discharge planning activities.
  • Participates in inter-disciplinary patient care conferences for skilled and extended care patients/residents.
  • Operates computer system to document record and retrieve appropriate information. Uses tools in EMR for consistency of case management.
  • Performs other duties as directed by a proper authority as indicated in an approved hospital policy.
  • Attend meetings as required.
  • Maintains professional growth and development to keep abreast of current social service standards.
  • Demonstrates knowledge and compliance to all CCH policies/procedures as well as state and federal regulations affecting discharge planning activities.
  • Consistently meets or exceeds customer expectations.

  • Actively practices interdisciplinary team orientated behaviors and communication.
  • Participates in hospital projects and public relations activities as requested
  • Participated in hospital committees and serves as a consultant to other departments as requested.
  • Consistently monitors and reports quality of care activities in accordance with CCH Quality Improvement Plan.
  • Demonstrates knowledge and compliance to all appropriate internal and external standards.
  • Completes annual competencies and education requires.
  • Consistently utilizes a holistic approach to patient care considering physical psychosocial spiritual educational safety and age related criteria.
  • Communicates effectively with people of diverse professional educational and lifestyle backgrounds.
  • Flexible with schedule to meet patient/family needs.
  • Supports Utilization Review activities with coverage when requested.
  • In times of high patient volumes may be assigned to duties in other clinical areas

Basic Clarke County Hospital (CCH) Performance Criteria

  • Demonstrates Clarke County Hospital Standards of Behaviors as well as adheres to hospital policies and procedures and safety guidelines.
  • Demonstrates ability to meet business needs of department with regular reliable attendance.
  • Employee maintains current licenses and/or certifications required for the position.
  • Practices and reflects knowledge of HIPAA DIA DNV OSHA and other federal/state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Qualifications:

Education: RN with current licensure Bachelors degree required within 3 years of hire. Preferred: BSN or bachelors degree in social work or a human services field.

Experience: Minimum of 1 year recent social service/care coordination related experience within a hospital preferred. Preferred: 3-5 years of experience in a clinical or community setting. Previous experience with community resources navigating patients through the healthcare continuum and working with disparate populations

License(s)/Certification(s): Valid drivers license when driving any vehicle for work-related reasons.

Knowledge/Skills/Abilities: Demonstrates excellent communication skills with patients families hospital staff physicians multidisciplinary teams community and other allied health care professionals. Comprehensive knowledge of community resources professional standards and resources. Must be able to read write and speak fluent English. Preferred: Bilingual in English and Spanish.

Shifts & Schedules: 32 hours per week; 4 days a week.
