Pay Grade: I
Under general supervision plans manages and evaluates social services programs. Administers and coordinates the services and program activities in dependency mental illness youth development or physical or developmental disability programs.
Facilitates and participates in the development review and evaluation of Individualized Family Service Plans for infants and toddlers with developmental delays/medical conditions; assists families in gaining access to early intervention services; coordinates multiple providers of services and collaborates with community agencies and providers; coordinates developmental evaluations and assessments.
This position will be housed at the Baldwin County Health Department in Milledgeville GA. Cases may be assigned from the 13 counties within the North Central Health District.
Bachelors degree in a related field from an accredited college or university and two (2) years of job-related experience; Two (2) years of experience at the lower level Social Svcs Prgm Coord 1 (SSP030) or equivalent position.
Note: Some positions require eligibility for licensure.

Employment Information
All approved teleworking employees must reside within the state of Georgia and depending on the responsibilities of the position must live within reasonable driving distance of the agencys primary worksite and be able to report to the office when required.
Current State employees are subject to State Personnel Board rules regarding salary.
DPH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. DPH will contact educational institutions to verify degree diploma licensure etc.
The candidate selected for this position may be subject to pre-employment drug screening and a criminal background check.
