The Birth Certificate Clerk obtains birth information from the patient by phone and by visiting patient rooms. Advises patient on state regulations regarding completion of birth certificates. Notarizes paternity affidavits in accordance with Louisiana law. Abstracts medical information from patient records and submits to the State. Submits birth information for every birth at Womans Hospital and maintains a record thereof. Assists patients in resolving problems regarding the birth certificate by communicating with local and state registrars. Performs various duties assigned by the department management according to policies and procedures and the mission of Womans Hospital.
