The social worker works under the supervision of a CZ Manager and is a member of the interdisciplinary Seniors Care Team responsible for providing service to seniors >65 years of age in the emergency department and community settings. They will provide a diverse range of interventions to clients and their families enabling them to achieve their optimal level of functioning and health. These interventions include psychosocial assessments adjustment therapy supportive therapy crisis counseling resource counseling discharge planning and case management. The Health Care Social Worker is actively involved in advocacy education program planning consultation and collaboration on behalf of individuals/groups within the institution and community. The role is multifaceted and dynamic requiring the Health Care Social Worker to work autonomously as well as collaboratively.
The social worker provides collaborative senior centered care across the health care continuum supporting the physical psychosocial and emotional well-being of all individuals (including patients families groups populations and communities). This position requires clinical coordination within the team and with community partners. The social worker is responsible for adhering to Nova Scotia Health policies and procedures and to the professional standards of their licensing body.

Please note:consideration may be given to those with a Masters of Social Work education and less than two year experience within a different pay scale.

$40.18 - $54.14 Hourly
Developmental (less than 2yrs experience) - $35.83 - $38.79 Hourly
