Reporting to the Manager Community Transitions the Occupational Therapist provides consultation assessment treatment and follow-up of clients in the occupational performance areas of self-care productivity and leisure for clients in the community throughout Central Zone of Nova Scotia Health. The objectives of treatment are to facilitate restore and/or maintain clients optimal level of function. The Occupational Therapist provides intervention to address clients occupational performance components (physical cognitive and/or affective). The Occupational Therapist provides education and support to clients families and caregivers. Excellent interpersonal communication skills are required. The Occupational Therapist is a member of the Community Occupational Therapy - Physiotherapy Team and liaises with Continuing Care multidisciplinary hospital care teams and community agencies and service providers.
