Job Description

The Family Support Specialist provides support and education to families with young children to provide a range of culturally competent home-based services in a variety of settings to meet them where they are including by phone virtual or in-person. This position is part of the Healthy Families America Program.


This is a full time role at 30 hours/week with scheduled hours occurring between 08:00am and 04:00pm based on family needs.


Essential Functions

  • Effectively initiate and maintain regular and long term contact and support with families.
  • Develop trusting relationships while assisting to strengthen the parent child relationship
  • Supports families in establishing goals and plans for attaining these goals
  • Identify and refer clients to other support services in the community; assists in making and attending health and human service appointments including but not limited to appointments related to educational and employment goals
  • Contact referrals in a timely manner and maintain weekly visits to all clients per the program requirements
  • Complete all assessments and evaluation paperwork within program guideline timelines. Maintains client records and completes all necessary documentation with program timelines
  • Ensures program follows all regulations regarding Home Visits
  • Participates in case conferences and collaborates with other service providers working with families
  • Uses reflective capacity to continue strengthening skills in delivering this program
  • Completes recordkeeping in accordance with Efforts to Outcome (ETO) and other agency requirements within required time frames and abide by agency confidentiality policy.

Other Job Functions

  • Properly applies Universal Precautions per agency policies and procedures
  • Demonstrates flexibility in the role by accepting other duties as assigned for which qualified
  • Demonstrates excellent communication and customer service skills
  • Identifies problems and offers constructive suggestions/solutions.
  • Adheres to Agency policies and guidelines regarding telecommunications use personal cell phone use time entry safety personal appearance confidentiality patient rights etc.
  • Completes annual education requirements and attends in-service programs as directed.
Requirements
  1. High school diploma required; Associate or Bachelors degree in human services or early childhood education preferred
  2. One to two years experience working with families with young children
  3. Knowledge of child development and positive parent child interactions
  4. Proof of completed COVID-19 vaccine series.

This is a Field Work position. Primary work location is in the community providing direct patient care. Not eligible for remote or work from home.

Monday Friday; 8:30 a.m. 5:00 p.m. Work assignments may include holidays. Occasional nights and weekend work may be required. Full time with benefits.
