Job Description
The Family Support Specialist provides support and education to families with young children to provide a range of culturally competent home-based services in a variety of settings to meet them where they are including by phone virtual or in-person. This position is part of the Healthy Families America Program.
This is a full time role at 30 hours/week with scheduled hours occurring between 08:00am and 04:00pm based on family needs.
Essential Functions

- Effectively initiate and maintain regular and long term contact and support with families.
- Develop trusting relationships while assisting to strengthen the parent child relationship
- Supports families in establishing goals and plans for attaining these goals
- Identify and refer clients to other support services in the community; assists in making and attending health and human service appointments including but not limited to appointments related to educational and employment goals
- Contact referrals in a timely manner and maintain weekly visits to all clients per the program requirements
- Complete all assessments and evaluation paperwork within program guideline timelines. Maintains client records and completes all necessary documentation with program timelines
- Ensures program follows all regulations regarding Home Visits
- Participates in case conferences and collaborates with other service providers working with families
- Uses reflective capacity to continue strengthening skills in delivering this program
- Completes recordkeeping in accordance with Efforts to Outcome (ETO) and other agency requirements within required time frames and abide by agency confidentiality policy.

- Demonstrates flexibility in the role by accepting other duties as assigned for which qualified
- Demonstrates excellent communication and customer service skills
- Identifies problems and offers constructive suggestions/solutions.
- Adheres to Agency policies and guidelines regarding telecommunications use personal cell phone use time entry safety personal appearance confidentiality patient rights etc.
- Completes annual education requirements and attends in-service programs as directed.
- High school diploma required; Associate or Bachelors degree in human services or early childhood education preferred
- One to two years experience working with families with young children
- Knowledge of child development and positive parent child interactions
- Proof of completed COVID-19 vaccine series.

This is a Field Work position. Primary work location is in the community providing direct patient care. Not eligible for remote or work from home.
Monday Friday; 8:30 a.m. 5:00 p.m. Work assignments may include holidays. Occasional nights and weekend work may be required. Full time with benefits.
