The Medical Records Clerk is responsible for the safekeeping of patient records managing incoming and outgoing medical and dental records requests scanning assigning and saving patient information and maintaining the integrity of the records storage area through a team-based approach. The Medical Records Clerk approaches his/her tasks in a team-based care fashion that supports patients and their families in self-management self-efficacy and behavior change.

Specific Duties:
- Delivers and retrieves patient records throughout SALUD
- Maintains the chart tracking system filing patient documents and records in the designated locations
- Carries out preliminary phases of planned record retirement
- All work is carried out in accordance with SALUDs policies and protocols
- Processes requests for records receives and processes all incoming requests for records following SALUD protocols
- Review each request to determine if authorized to release records and the adequacy of patient information
- Prepares charts and files all patient information in a designated location
- Works within a team atmosphere to complete some tasks
- Promotes public relations through prompt and courteous service
- Maintains respect for privacy while adhering to confidentiality in all phases of work
- Works in conjunction with other Medical Providers Dental Providers Behavioral Health Providers and all other staff to ensure that the patients obtain the highest standards of integrated healthcare
- Acts as a patient advocate for our vulnerable populations by ensuring that patients are fully informed and comfortable with their visit to SALUD
- Follows procedures for the electronic health records system
Qualifications :
Education:
- Graduation from an accredited high school or equivalent
Experience:
- One year of general office experience is desired but not required
Knowledge Skills and Abilities:
- Ability to file both alphabetically and numerically with no errors
- Ability to detect errors in patient identification and make corrections
- Knowledge of medical/dental terminology preferred
- Ability to read understand and follow oral and written instructions
- Bilingual English/Spanish preferred
- Microsoft Programs: Excel Word and Outlook
- Must work well with the other patient care team members
- Sensitivity to low-income and ethnic minority communities is a must
