Job Description
We are in search of a dedicated Nurse Practitioner to become part of our Long-term care group!
A fast-growing team is in search of a Primary Care Nurse Practitioner to see geriatric patients in nursing homes located in the five towns.
Requirements:
- Valid NP license in the state of NY
- Nursing Home experience- a must
- Credentialed with Medicare- strongly preferred
Duties will include:
- Managing all Primary Care related consults
- History and physical exam
- Medication review
- Prescribe medication and drugs
- Check in with patients who have acute issues and establish a follow-up plan
- Reach an informed diagnosis based on medical knowledge and the patients medical history
Schedule:
- 8-hour shifts
- Working days: Sunday Tuesday Thursday Friday and Saturday
