Job Description

Our client is seeking a compassionate Home Health Clinical Support RN to join their extraordinary care team. The position offers a competitive salary and benefits package including paid time off 401k plan and industry-leading benefits program. The role involves providing care to patients in Chicago IL and surrounding areas with a focus on delivering outstanding care in a team-based environment. Key Responsibilities Provide all levels of care to the patients Support patients in Chicago IL and surrounding areas Meet critical deadlines for charting Self-manage time and schedule Demonstrate experience with tablets mobile phones and EMR software Schedule and Shift Details Monday through Friday on the day shift; no weekends on-call or holidays

Interview Process: - Phone Screen w/ HR - In-Person with HM - Offer

Requirements

required

High School/GED

Epic Home Health (1 years)

2 years of experience in ANY of the following:
- Home Health Care
- Home Health
1 years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- TruChart EHR

Valid Registered Nurse (RN) in the state of Illinois (IL)
Valid Regular Driver License (DL)

Are you okay with the required travel for this position

Master's degree or higher Healthcare Setting (1+ years) Valid Physical Therapist in the state of Illinois (IL) Valid Regular Driver License (DL) Local travel to residential homes Inpatient PT experience preferred OASIS & HCHB highly preferred, but can be trained on this

Education

Master Degree or higher Valid Physical Therapist in the state of Illinois (IL)
