Description:
Destiny Healthcare Services is seeking a dependable and compassionate Field Supervisor to join our team. This role is responsible for conducting home visits for clients enrolled in the Community Care Program (CCP) to ensure quality of care and compliance with care plans.
Key Responsibilities:
Conduct routine home visits to CCP clients to monitor service quality and caregiver performance
Complete and submit visit reports and documentation in a timely manner
Communicate with clients caregivers and office staff to address any concerns or needs
Ensure that services provided are consistent with the care plan
Report any client changes or safety concerns to the administrative team
Assist with new client intakes and reassessments as needed
Requirements:
Clean background check (required)
Valid drivers license
Excellent communication and organizational skills
Ability to work independently and manage a flexible schedule
Prior home care or healthcare experience is a plus
Bilingual (English/Spanish) is a plus but not required
