About the Job
Seasons is a comprehensive behavioral health center offering a broad range of psychiatric and behavioral health services to communities in the Northwest Iowa region. Were currently seeking a full-time Care Coordinator. The Care Coordinator will help coordinate service provision for providers facilitating communication between providers and ensuring individuals receive the appropriate care for their individual needs. This position could be based in Spencer Sheldon Sibley Spirit Lake or Sioux Center IA.
Salary Range
$22.00-$26.00 per hour (Commensurate with experience)
- A Bachelors degree in social work human services or a related field is required
- Experience providing care coordination services for individuals with behavioral health concerns is preferred but not required
- An active Iowa Bachelors of Social Work (BSW) license is preferred but not required
