Under supervision provides persons families or vulnerable populations with the psychosocial support needed to cope with chronic acute or terminal illnesses and physical or developmental disabilities. Provides patient education and counseling advises caregivers and makes necessary referrals for other social services.
Additional Duties:
The Ryan White Clinic is seeking a Case Manager/Social Service Provider-Non Licensed. The Medical Case Manager will work closely with clients living with HIV/AIDS who have multiple psychosocial and/or health related needs. Medical Case Management services is a strength-based approach to service that includes treatment adherence counseling coordination and follow-up of medical treatments client advocacy and assistance in obtaining housing financial support social support and any other needed service that is available. The goals of Medical Case Management are to insure that clients with HIV/AIDS have timely access to comprehensive medical care and social services; prevent disease transmission and delay of HIV progression and to promote and support client independence and self-sufficiency. The Medical Case Manager will be supervised by the Healthcare Program Consultant Supervisor. Prior working experience with the HIV/AIDS community is preferred.Minimum Qualifications
Bachelors degree in a related field from an accredited college or university and two (2) years of experience in social service delivery. Additional Information

DPH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. DPH will contact educational institutions to verify degree diploma licensure etc.
