Erie Family Health Centers a nationally recognized top workplace with 13 sites in Chicago and suburbs is looking for a valuable addition to the Lending Hands for Life Team. The Lending Hands for Life non-medical case manager provides non-medical case management services to HIV clients at Erie Family Health Centers (EFHC). LHL Non-Medical Case Manager works with providers LHL team members and other staff at Erie to coordinate the delivery of services to clients assist with access to care and facilitate the appropriate use of services.

This role is a hybrid job. At Erie a hybrid job combines working onsite at a specific Erie location and working from home from an approved work schedules can vary depending on the role and needs of the organization.
Main Duties & Responsibilities
Manage a caseload of medically stable HIV clients in accordance with agency policies and procedures.
Provide non-medical case management services including coordination of medical services resource linkage and benefits enrollment and assistance to HIV clients.
Work regularly with other members of the HIV Team including medical providers. Attend team meetings case manager meetings CQI meetings site meetings and weekly case conferences with each provider.
Document in the electronic medical record and state-database for all communication related to client care
Work on projects and initiatives to improve the quality of case management and quality of the HIV program including outreach and low barrier care.

Education
Required Non-Medical Case Managers must meet one of the following education criteria:
Bachelors degree in Social Work (BSW) from an accredited college or university with licensure preferred;
Bachelors degree in related human service social science or health education field from an accredited college or university;
Associates degree in a related human service social science or health education field from an accredited college or university plus 2 years of case management or related experience; or
Demonstrated knowledge and at least 4 years of work experience equivalent to the academic credentials outlined above.
Skills and Knowledge
Required:
Familiarity with issues affecting individuals living with HIV.
Verbal and written communication skills.
Computer proficiency is required.
CPR certification required.
Preferred:
Bilingual fluency (English/Spanish) is strongly preferred (May be required to assist or work with non-English/Spanish-only speaking patients and/or guests.)
Experience:
Required:
The ability to relate to people of diverse backgrounds cultures races sexual orientation and gender identities or expressions.
Preferred:
At least 2 years of direct service experience working in HIV service provision preferred.
