Carolina Outreach is looking for a compassionate driven and team-oriented Qualified Professional to join our Community Support Team. In this vital full-time position youll work directly with individuals living with mental health and substance use challenges helping them build the skills and access the resources they need to lead fulfilling independent lives.
As a Community Support Team member youll be at the heart of client careoffering crucial services in real-world settings. From crisis intervention and service coordination to psychoeducation and skills training youll be delivering hands-on support that truly makes a difference. Youll serve as a bridge between clients and the services they need ensuring they are empowered to thrive within their communities.
Every day will bring new opportunities to collaborate with professionals across agencies create individualized therapeutic interventions and use community-based activities to achieve clinical goals. Youll work closely with clients their families and natural supports to develop and implement Person-Centered Plans that reflect each individuals unique needs and aspirations.
