Job Description

Pay Grade/Pay Range:Minimum: $27.21 Midpoint: $35.35 (Hourly N9)

Department/Organization:208401 UMC Clinical

Normal Work Schedule:Monday Friday 8:00am to 4:45pm

Job Summary:The RN I provides professional nursing care to patients. Reviews physicians orders for accuracy and authorization prior to implementing them. Prescribes and initiates nursing treatment through direct assisted or delegation to other staff.

Additional Department Summary:Performs liaison duties for University Medical Center (UMC) clinic patients. Responsible for organization and execution of care management services including previsit planning care gap analysis and postvisit care coordination. Proactively manages Annual Wellness Visits at UMC clinics. Refers patients into the appropriate external care management programs. Communicates and connect with patients inperson by phone or electronically as needed.

This position is focused on patient support and education aiming get patients more active in their healthcare by providing a better understanding of their illnesses and assisting them in becoming more involved in coordinating their postvisit care.

Required Department Minimum Qualifications: Active RN license in the State of Alabama in good standing. One (1) year of RN nursing experience. Must have a current CPR certification and have used an electronic health record system in a previous learning or employed role.

Final candidates must submit to and successfully pass a postoffer drug screen.

Skills and Knowledge:Comprehensive clinical assessment skills in the areas of population and chronic disease management. Familiarity with current clinical practice standards for ambulatory and primary care. Ability to prioritize tasks and thrive in a busy multipractice environment. Familiarity with community and commitment to learning and understanding through on the ground networking community assessment etc.

Experience providing patient education using the Teach Back Method and teaching selfmanagement skills. Experience working with vulnerable populations (elderly minorities behavioral health socioeconomically disadvantaged underinsured or those with certain medical conditions). Ability to navigate ambiguity with the aid of structured problemsolving techniques.

Exceptional communication skills both written and oral. Ability to positively influence others with respect and compassion. Strong work ethic built on a foundation of proactivity collaboration and teamwork. Committed to the practice of inquiry and listening. Embraces electronic health records. Demonstrates curiosity of learning and receiving critical feedback to further growth and development.

Preferred Qualifications:Bachelor of Science in Nursing (BSN) preferred. Experience in care coordination care/case management quality improvement ambulatory care or community health.
