The program provides both short and long term transitional support to students whose learning has been affected by serious mental health and/or medical challenges. The goal is to reintegrate students back into school by providing clinical skillbased psychological and academic support. QualificationsExperience or familiarity with Bridges programs and/or other transition programs preferredExperience working with students with and without disabilities in the high school settingExcellent organizational technology and communication skillsAbility to collaborate with colleagues and function independently and as part of a teamDemonstrate the same high level of ethical behavior and confidentiality of information about students as is expected of all educatorsBachelors Degree in Psychology Social Work or related fieldResponsibilities:Work in collaboration with the School Psychologist and Special Education Teachers to meet the educational and social/emotional needs of studentsProvide direct academic and social/emotional support to students Work collaboratively with educators in inclusion classrooms Create and foster opportunities for generalization and maintenance of strategies and skills Foster career exploration and ensure students are able to progress toward independenceImplementation of positive behavior support plans if necessaryCollect observational data for the purpose of progress monitoring if necessaryAssist students safe transitions to/from locations and environments within and around the school if warrantedParticipate in program meetings and specialized training related to specific and individualized needs of the studentMaintain student privacy and confidentialityRespond appropriately to emergency situations as per standing orders and other policies and proceduresDisplay a positive role model for students at all times Provide continuous feedback to studentsReview progress on IEP goalsConsistently and effectively interact and communicate with program supervisors school psychologist educators and studentsAssume other responsibilities as assigned by Administrators
