HOURS: Mostly 6:00am 2:30pm OR 1:00pm 9:30pm
Primary Function/General Purpose of PositionThe Care at Home Specialist supports the daytoday operations of the Care at Home programs including Remote Patient Management (RPM) and Hospital at Home. The Care at Home Specialist is responsible for the program development logistics inventory and servicing of telemonitoring emergency response equipment and additional supplies & equipment necessary for all programs. The Remote Monitoring Equipment Specialist will be the technology subject matter expert to ensure an optimal patient experience. The Care at Home Specialist will coordinate and communicate with vendors customer service departments and technicians as well as the Remote Patient Management Program and Hospital at Home teams as needed. The Care at Home Specialist is responsible for inventory control that assures all equipment is retrieved in a timely manner when services are discontinued as well as cleaning and inspecting the equipment before redistribution. The Care at Home Specialist is an advocate of the programs and will support continuous improvement that assures success and sustainability for the community we serve.

Working Conditions/Physical Requirements
Frequent bending stooping standing stretching walking and sitting. Requires full range of body motion including handling and lifting equipment manual and finger dexterity eyehand coordination and safe driving skills. Occasionally lifts and carries items weighing up to 50 pounds. Requires frequent talking in person and on the telephone. Vision correctable to within normal limits. Hearing required (hearing aid permitted). Requires fine motor coordination operation of motor vehicle (or available transportation) operation of medical and office equipment or machinery. Other: access to a variety of clinical locations. Requires traveling locally. Exposure to environmental factors (weather unsafe home environments) chemicals solvents bactericidal agents body fluids and animals.
