Job Description
A community Primary Care Clinic near San Jose CA is seeking a Licensed Social Worker to join their team. Details are listed below.
Pay Rate: $80000 $110000 / Annually
Licensed Social Worker Opportunity:
- Fulltime directhire position
- Schedule: Monday Friday 8 AM 5 PM
- No oncall or afterhours work required
- LCSW or LMFT will report to the Counseling Director
Licensed Social Worker Responsibilities:
- The Licensed Social Worker will oversee case management by developing treatment plans and connecting clients with resources
- The Licensed Social Worker works alongside psychiatrists physicians and other medical professionals to ensure clients receive the services they need
- Help clients understand their own strengths and areas for growth
- Assist clients in developing plans using their strengths to overcome obstacles
Licensed Social Worker Qualifications:
- Master s Degree in Social Work or related fields
- LCSW or LMFT in CA Required
- Bilingual in English and Spanish preferred
