Duties:
- Performs initial and ongoing assessments of patients condition for patients in an inpatient setting as well as outpatient ambulatory case types
- Establishes revises and evaluates a plan of care which is appropriate to problems identified and involves the patient/family
- Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient
- The physical therapist evaluates and assesses participants to determine mobility level and other functional abilities
- Provides and recommends therapeutic exercise gait training ambulation etc.
- Conducts initial assessment and periodic reassessment (minimally every 6 months) of all participants physical condition and functional status
- Assessments must be completed prior to scheduled team meeting
- Communicate participants changes to team
- Develops Plan of Care develops treatment programs when indicated sets goals and provides treatment
- Records and signs treatments in participants chart records signs and dates Progress notes
- Assesses the need for adaptive and durable medical equipment specialized shoes and prosthesis and makes recommendations to the Interdisciplinary Team
- Actively functions as member of Interdisciplinary Team
- Attends staff meetings and takes part in participant care planning
- Communicates any changes in participant condition to team members
- Demonstrates active participation in QI processes

Qualifications:
- Demonstrates eligibility to work for any employer in the U.S.
- Must be licensed in the State of Michigan and/or eligible for licensure as required by the state practice guidelines
- Masters Degree or Doctorate Degree in Physical Therapy required; Must be a graduate of an accredited physical therapy curriculum
- Current Basic Life Support certificate or ability to obtain within 45 days of the date of hire
- Previous experience preferred within a residential environment
- Must demonstrate excellent communication skills both verbal and written English communications for safe and effective patient care and to meet documentation standards
- Ability to multitask and move between responsibilities in a fluid manner
- Able to work in a fastpaced and stressful environment while maintaining positive energy
- Willingness to participate in goalsetting and educational activities for professional advancement
- Able to work compassionately with patients and coworkers to exhibit patient care second to none
- Exudes respect and flexibility to impact the workplace in a positive manner
- Detailed oriented conscientious and committed to precision in work results
- Ability to perform to a high level of accuracy
- Friendly empathetic & respectful
- Must be reliable in work results timeliness and attendance
