What you bring to SBHG (qualifications):
- 4 years of experience in a social service field OR
- Associate Degree plus 2 years of experience in a social service field OR
- Bachelors degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service
- Valid California Drivers License
How you will make a difference (job overview):
This position has primary responsibility for case management services to clients and their families referred through Stars Transition Age Youth (TAY) Program (TAY)
Division/Program Overview:
- Outpatient communitybased program that serves youth age 1825 with serious and persistent mental health issues
- Program provides intensive case management outreach and engagement medication support as well as mental health counseling peer groups and supportive services to assist TAY transition to adult life.
- Services offered are trauma informed and culturally responsive
