22.36 26.83 USD Per Hour

Working Location:

San Leandro: San Leandro Center (InPerson Position)

Working Hours/Shift:

MondayFriday AM Shift AM Shift

What you bring to SBHG (qualifications):

  • 4 years of experience in a social service field OR
  • Associate Degree plus 2 years of experience in a social service field OR
  • Bachelors degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service
  • Valid California Drivers License

How you will make a difference (job overview):

This position has primary responsibility for case management services to clients and their families referred through Stars Transition Age Youth (TAY) Program (TAY)

Division/Program Overview:

  • Outpatient communitybased program that serves youth age 1825 with serious and persistent mental health issues
  • Program provides intensive case management outreach and engagement medication support as well as mental health counseling peer groups and supportive services to assist TAY transition to adult life.
  • Services offered are trauma informed and culturally responsive

In accordance with California law the grade for this position is 22.36 33.54. Placement within the grade is determined based on experience internal equity and other factors permitted by law.
