SchoolBased Success Coach (Case Manager) Las Cruces NM
Make a Difference in Youth & Family Services!
Families and Youth Innovation Plus is looking for a SchoolBased Success Coach (Case Manager) to join our team. The SchoolBased Success Coach provides specialized screening support resource navigation and case management services to assigned and/or referred students within their assigned school(s) . They also provide positive youth development SEL skills development enrichment and coaching to benefit assigned youth and families. Success coaches ensure that program participants and their families receive LVP services in a timely appropriate efficient and coordinated fashion.
What Youll Do:
- Coordinate services between providers and clients
- Provide youth case management and skillbuilding
- Maintain records and complete intake documentation
- Conduct field visits and facilitate youth activities
- Attend community meetings relevant to program and clients
What You Need:
- Bachelors degree in Social Services Social Work Counseling Education Criminal Justice or similarly related field
- Two (2) years experience working with at risk youth or children in an educational behavioral health or similar setting.
- Excellent communication skills both written and verbal
- Exhibit skills in comprehending interpreting and completing delegated tasks efficiently
