ENTRY LEVEL SALARY:PG11 FULLTIME

OFFICE LOCATIONS: Various Housing Sites in Delaware

These nonState positions are responsible for case management work advising and assisting residents of subsidized housing to develop or obtain the skills and resources necessary to meet personal social and economic needs. Incumbent in this job functions under the direct supervision of a technical or administrative supervisor. Work involves but is not limited to aiding and advising residents to provide assistance in goal setting; verifying and updating resident status on quarterly basis; providing updated information and referral services to community resources; interaction with other departments and outside resources; ensuring overall compliance with the Moving To Work Program in a cost effective timely and productive manner.

PRINCIPLE ACCOUNTABILITIES

  • Interviews client family and/or professional sources to determine clients needs for a variety of social services which may include employment job search and basic skills remediation.
  • Develops a comprehensive costeffective service plan utilizing inhouse sources referrals contracted providers and/or institutions.
  • Conducts ongoing monitoring and assessment of service delivery for optimum quality and efficiency and authorizes and recommends changes and adjustments as needed.
  • Maintains comprehensive case records summaries statistical and narrative reports and completes required forms.
  • Establishes effective working relationship with clients family community and professional sources.
  • Provides direct casework service through individual/group conferences advisingand/or site visits.
  • Supplies information and responses to inquiries regarding services for clients family community and government agencies.

MINIMUM QUALIFICATIONS

Applicants must have education training and/or experience demonstrating competence in each of the following areas:

Possession of an associates degree (or higher) in a social science program area such as sociology criminal justice counseling psychology human services or directly related degree.

OR:

  1. Six months experience in case management which includes assessing planning developing implementing monitoring and evaluating options and services to meet an individuals human service needs and ensuring program compliance.
  2. Six months experience in making recommendations as part of a clients service plan such as advising or determining eligibility for health or human services/benefits.
  3. Six months experience in interpreting laws rules regulations standards policies and procedures.
  4. Six months experience in using an automated information system to enter update modify delete retrieve/inquire and report on data.
  5. Six months experience in narrative report writing.
  6. Successful completion of a criminal background screening
