Location: Oxford Connecticut
A respected and growing healthcare facility in Oxford CT is seeking aBoard Certified or Board Eligible Family Medicine Physician to join its dedicated primary care team. This is a fulltime clinicbased position offering a structured schedule and a supportive environment ideal for both experienced providers and recent residency graduates.
The clinic is modern and wellequipped with advanced EMR integration onsite diagnostic support and care coordination services. You will work with a collaborative team of physicians nurse practitioners medical assistants and behavioral health professionals all committed to delivering patientcentered comprehensive care to a diverse community.
Key responsibilities include managing chronic conditions conducting preventive screenings performing wellness visits and promoting healthy lifestyle choices for patients of all ages. You will coordinate with specialists as needed and participate in quality improvement efforts and population health initiatives within a patientfocused care model.
