RESPONSIBLE TO: Administrator
FUNCTION: Develop organize and manage the daytoday operation of the Adult Day Health Care Program including development and maintenance of liaisons with other health care providers and service agencies.
Determine patient eligibility for program services.
Coordinate manage and supervise direct and contracted services and compliance with Federal and New York State regulations for operation of the Adult Day Health Care Program.
DUTIES AND
RESPONSIBILITIES: Develop and maintain public relations with area health care providers staff members patients families other service providers and the members of the community.
Participate with community agencies to ensure an orderly admission/discharge planning process.
Conduct a comprehensive assessment of patients and families in order to determine health care needs and develops a plan of care for each registrant in the program.
Provide appropriate job orientation and instruction of new personnel including service contract personnel.
Coordinate the scheduling of patient care services provided by the staff employed directly or through contract with the Adult Day Health Care Program.
Provide direct services to patients of the Adult Day Health Care Program.
Has overall responsibility for all patient services and the quality of care delivered within the Adult Day Health Program.
Understand and interpret New York State regulations for the Adult Day Health Care Program.
Prepare and maintain all policy and procedural manuals and submit all required reports to assure compliance with regulations.
Assist with the collection of statistical data for Adult Day Health Care registrants and services.
Make and communicate recommendations for improvement in the delivery of services or the addition of new services.
Handle all registrants concerns and complaints according to policies and procedures.
Maintain nursing practice and activities in accordance with professional standards.
Other related duties that may become necessary or as directed by administration.
MINIMUM JOB REQUIREMENTS:
An Associates Degree in Social Work preferred or a minimum of two (2) years working in a nursing home or other community based health care program in a supervisory or administrative capacity or its equivalent.
Must be available to work more than eight (8) hours per day or more than forty (40) hours per week.
