Your Role
As the Indiana Program Manager you will be responsible for empowering consumers to live their best lives at home and in the community. This dynamic role involves building meaningful relationships with consumers families and team members to understand and advocate for individual needs and desires. Youll create personalized support plans attend quarterly progress meetings and prepare insightful monthly reports. Additionally youll hire train and support staff to ensure highquality personcentered care. Responsibilities
  • Communicates well with and develops a relationship with consumers families staff members case managers and all other members of the team to determine the likes interests strengths wants and needs of each consumer.
  • Completes and/or analyses needed assessments which may include Personal Outcome Measures Risk Assessment Rights Assessment Support Assessment etc. to develop the MY LIFE document that will help the team create the PCISP with meaningful outcomes for each consumer.
  • Advocates for each consumers wants and needs during all aspects of the job.
  • Attends each consumers quarterly meeting to discuss the progress on outcomes and address any changes the consumer would like to see with services.
  • Prepares monthly summary reports in a timely manner that detail the services provided identifies progress on outcomes and shares them with the team through the state portal system.
  • Provides medical financial and safety support as requested to ensure the consumers wants and needs are met.
  • Hires and trains staff to ensure they are providing person centered opportunities to support the consumers wants and needs develop relationships protect their rights and safety and advocate for each of them.
  • Completes staff evaluations yearly to identify strengths and areas for improvement and develops goals to support staff in developing the skills necessary to meet the needs of each consumer they support.
  • Completes all billing and reporting requirements in a timely manner.
  • Ensures quality services are provided that meet the wants and needs of the consumers while also meeting the regulatory standards of DDRS and CQL.
Qualifications
  • Excellent verbal and written communication skills
  • Ability to effectively work with consumers outside providers and family/guardians
  • Organizational and Time Management skills
  • Excellent computer proficiency (MS Office Word Excel and Outlook)
  • At least three years of experience working with individuals with disabilities
  • Ability to work independently and to carry out assignments to completion within parameters of instructions given prescribed routines and standard accepted practices
  • Must maintain a valid drivers license and meet agency driving requirements
  • Bachelors degree in Human Services field a plus
