We operate 18 residential group homes along the I5 corridor from Portland to Eugene.
As a Medical Home Site Manager you will:
Work in partnership with the assistant site manager and nurse manager to support a team of health crisis technicians Registered Nurses (RNs) and respiratory therapists (RTs) in providing exemplary care to medically fragile individuals supported in the home.
Manage staffing schedules daily assignments review community outing requests and financials such as petty cash state sponsored credit card (SPOTs card) and individual accounts.
Work collaboratively with other agencies and community partners to promote individual choice and personal growth.
A valid drivers license and acceptable driving record are required for this position.
Four years of lead work supervision or progressively related experience.
OR
One year of related experience and a bachelors degree in a related field.
We are looking for candidates with:
Experience managing medical homes that support medically fragile individuals.
Experience using Microsoft Word Excel Outlook and TEAMs.
Experience using an electronic medical record (EMAR).
Experience which has provided you with HIPPA knowledge.

You must have a valid license to drive and you must have and maintain an acceptable driving record to transport individuals by state van or car.
You will work in a residential medical home environment staffed seven days a week 24 hours a day.
Your work schedule will be Monday through Friday; however your schedule may require working some evenings and weekends as needed. The nurse manager will assign your work schedule based on operational needs.
You will interact daily with a wide range of people including some who have experienced trauma and may exhibit challenges in controlling their anger or other unexpected emotions or behaviors.
If selected as a finalist we will conduct a criminal history and background check. Adverse criminal history or background check may lead to disqualification.
The Oregon Department of Human Services does not offer visa sponsorship. Within three days of hire you will be required to complete the US Department of Homeland Securitys I9 form confirming authorization to work in the United States. ODHS will use EVerify to confirm that you are authorized to work in the United States.
The Department of Human Services is a drug free workplace. All applicants will be subject to drug testing for Narcotics Methamphetamine and other controlled substances.
Your driving record must meet the qualifications of the ODHS SACU Driving Policy and will be considered unacceptable for offenses including but not limited to any misdemeanor or felony traffic offense in the last 24 months.
If you are offered a position you will attend a threeweek paid inperson training at our central office in Salem and you must become certified during this New Employee Orientation in the following:
First Aid/CPR Certification Requiring you to complete compressions in a kneeling position.
Oregon Intervention System Certification (OIS) Requiring you to physically hold an individual for safety while maintaining an approved restraint on clients which may involve quick movements changes in physical positioning and verbal communication with coworkers.
Medication Administration Certification Requiring you to demonstrate competency in completing doctors orders appropriate passing of medications to clients appropriate documentation and appropriate procedures for emergencies.

This position is part of the Police and Fire pension plan which allows you to retire with full benefits after 25 years of service.
