Summary of job:

As the Integrated Care Team Leader you will oversee the provision of medical and preventative support services within the BCC Integrated Care Clinic assist in coordination of medical services health navigation and provide health education to all members of the patients integrated care team PCP and family and other natural supports. In this position you will work in collaboration with the Department Leader to assure a high standard of care in integrated medical health and wellness services. You will provide training education and support on medical health and wellness concerns throughout the organization. You will work directly with patients with complex needs clinical staff onsite and offsite physicians family and other natural supports. This position involves communitybased work to assist clients in coordination of care for physical health and psychiatric services support for onsite care of clients with SMI and engagement with community resources to help clients with SMI address their social determinant of care needs.

YOU WILL

• Oversee and implement medical care and preventative care services for patients with behavioral health and acute and chronic health conditions or at risk for such conditions. Provide direct care education and support to patients with complex physical and behavioral health needs.
• Oversee the daily operations of the Integrated Medical Department.
• Supervise Medical Assistants Medical Care Coordinator(s) and other staff assigned within the medical clinic.
• Hire orient and evaluate medical support staff.
• Participate in the onboarding and orientation of new psychiatric and medical providers. Etc.

TO QUALIFY FOR THE POSITION YOU WILL NEED

• Licensed Practical Nurse (LPN) active in the state of Pennsylvania required
• At least two (2) years of experience in a behavioral health setting required.
• At least one (1) year experience leading others.
• Ability to work 40 hours per week within agency hours of operation which are 8am8pm MonThurs 8am4pm Friday with Saturday hours when applicable.
• A valid PA Drivers License and the ability to drive travel and provide communityoriented services.

PREFERRED CANDIDATES WILL

• Registered Nurse (RN) Preferred
• Be Bilingual: English/Spanish preferred.
• Have Knowledge of /or experience with traumainformed approaches preferred.
• Basic Life Support certified.

PHYSICAL REQUIREMENTS:

• SEEING: Must be able to see well enough to read reports and use computer.
• COMMUNICATING: Must be able to communicate well enough to patients vendors coworkers and physicians.
• MOBILITY: Must have mobility to move about the agency. Must have mobility to move about the community.
• LIFTING/PULLING/PUSHING: Must be able to lift up to 10 pounds.
• OTHER: Must be able to write type and use technology.

5. This position may require you to operate BCCs company car or transport clients so you must provide a 10Year Motor Vehicle Record (MVR) and a valid Drivers License
