JOB DESCRIPTION


JOB TITLE: Case Manager/Advocate

REPORTS TO: Lead Care Manager

STATUS: Nonexempt

LOCATION: Domestic Violence Resource Center/Advocacy Center


JOB SUMMARY:

The Case Manager/Advocate is responsible for the provision of case management services at Lauras House Emergency Shelter. The Case Manager plays a vital role in enhancing community awareness and collaboration regarding domestic violence issues. The Case Manager will conduct outreach initiatives to inform stakeholders about available resources and services ensuring timely intervention and assistance for individuals affected by domestic violence. By fostering these essential connections the Case Manager will contribute to a holistic approach in addressing the needs of survivors promoting safety healing and empowerment within the community.


JOB DUTIES:

  1. Develop and implement a case management plan addressing the physical financial emergency medical vocational educational emotional and housing needs of the client and their children.
  2. Complete intake assessments with Lauras House clients living in the community or in residence at the Emergency Shelter/Transitional Living Center via telephone and inperson.
  3. Maintain accurate current and complete case files for each client.
  4. Enter statistical and descriptive data for contract and other reporting requirements as needed.
  5. Provide crisis intervention by telephone or in person as needed.
  6. Provide clients with transportation as needed.
  7. Facilitate client groups as needed.
  8. Supervise volunteers as needed.
  9. Actively engage in community outreach efforts to help raise awareness about domestic violence and Lauras House services.
  10. Establish and maintain working relationships/partnerships with other agencies to better serve Lauras House clients.
  11. Represent Lauras House at networking events.
  12. Enforce the Emergency Shelters policies and procedures.
  13. Assist other departments at the shelter as needed.
  14. Other duties as assigned.

ADDITIONAL RESPONSIBILITIES:

  1. Maintain the confidentiality of client staff volunteer and agency information. Exceptions include information pertaining to child abuse elder abuse danger to self or others or in response to an authorized release of information.
  2. Maintain appropriate boundaries with clients and other staff.
  3. Represent the agency in a positive professional manner at all times.
  4. Maintain a tidy organized work area.
  5. Participate in agency functions as a participant or presenter as needed.
  6. Evening and weekend hours as needed.

EDUCATION AND EXPERIENCE:

  1. Bachelors degree in social sciences human services or related field or equivalent experience required.
  2. One or more years of legal advocacy or case management experience preferred.
  3. 40hour domestic violence training as required by law. (Can be completed after hire).

KNOWLEDGE SKILLS AND ABILITIES:

  1. Bilingual Spanish preferred.
  2. Excellent oral and written communication skills.
  3. Basic computer skills.

JOB QUALIFICATIONS:

  1. Valid drivers license reliable transportation and appropriate insurance.
  2. Completion of Live Scan.
  3. Employee is occasionally required to stand walk sit use hands to finger handle or feel climb or balance stoop kneel crouch or crawl talk or hear and taste or smell. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by the job include close vision distance vision color vision peripheral vision depth perception and ability to adjust focus.
