NOW HIRING: Community Health Worker (Beautiful Beginnings Section)
Grantfunded

The Community Health Worker (CHW) will work closely with individuals and their infant(s) in the community during pregnancy the postpartum period and/or up to the childs third birthday. The CHW will work in tandem with a Public Health Nurse to provide personalized communitybased care to these clients. The CHW will assist clients in finding community resources which reduce their barriers to care and help ensure that the client is knowledgeable about their health and the health of their infant(s). This position is grant funded through a Community Partnership Grant.

Duties Include:

Client outreach and enrollment activities with pregnant and postpartum individuals.
Conducts home and community visits with families and infants from diverse cultural socioeconomic educational racial and ethnic backgrounds.
Identifies needs based on the social determinants of health and provides appropriate resources referrals and assistance.
Educates clients about the importance of preventative health care; provides basic education to ensure an understanding of their health status and conditions.
Accurately documents all client contact in an Electronic Health Record System including using mobile technology for partial remote work and on home visits.
Attends local events and disseminates health related information throughout the community.
Other related duties as assigned including participating in emergency preparedness or response activities.

Requirements:

Community Health Worker in Training

High school diploma or equivalent
Must be certified as a Community Health Worker within the first year of employment
Ohio Drivers License
Bilingual communication preferred.
Experience in community health coordination/navigation preferred.

Community Health Worker I

High school diploma or equivalent
Knowledge and proficiency using an Electronic Health Record system
Community Health Worker Certification
Ohio Drivers License
Bilingual communication preferred.
Experience working in community health social work public health or community education preferred.
OBGYN prenatal and/or postpartum experience preferred.

Community Health Worker II

High school diploma or equivalent
3 years experience as Community Health Worker or working in community health public health or community education setting.
Knowledge and proficiency using an Electronic Health Record system
Community Health Worker Certification
Ohio Drivers License
Bilingual communication preferred.
Associates degree in public health public administration social work or other human services related field preferred.
OBGYN prenatal and/or postpartum experience preferred

Hiring Salary:

Community Health Worker in Training: $19.05/hour to $21.91/hour.
Community Health Worker I: $20.30/hour to $23.35/hour.
Community Health Worker II: $22.63/hour to $26.03/hour.
