Job Summary
Assesses the biopsychosocial needs and abilities of residents and families providing a range of Social Work services interventions and education to meet their needs and enhance abilities. Utilizes personal family and community resources to achieve and maintain an optimal level of physical mental social wellbeing and quality of life for the residents and families. Ensures the following is completed: biopsychosocial assessments care planning counseling advocacy for resident and family members; safe and appropriate discharge planning.
Essential Functions
- Completes comprehensive biopsychosocial assessments of residents total needs and abilities including evaluation of social mental emotional financial environmental and family needs. Develops care plans to meet identified needs and goals of assigned residents and families with interdisciplinary care planning team.
- Provides advanced social work services including: casework safe and appropriate discharge planning counseling and advocacy for residents and families. Maintains current knowledge and list of community resources and services. Refers and links residents and families to appropriate community resources and services.
- Acts as a resident advocate to protect the individuals rights and as a liaison between the facility community resident and family. Keeps residents and appropriate responsible parties/surrogate decision makers informed of resident problems status and needs per interdisciplinary meetings.
- Provides leadership for all staff in addressing resident mental health issues through interdisciplinary assessment intervention and inservicing. Coordinates timely mental health services including but not limited to: referral for psychiatric evaluation and counseling assures recommendation from psychiatrist psychologist behavior and pain management are followed up on refers appropriately to behavior and pain management.
- Assures all departmental documentation assessments and reports complies with regulation and facility protocol LTC population examples: quarterly progress notes yearly updates resident rights advance directives etc.
- Participates in assigned team meetings to enhance care and quality or services. Meets with appropriate facility staff concerning patient issues particularly related to cognition mood behavior and discharge planning issues.
- Demonstrates commitment to professional growth and competence by completing required mandatory inservices attending departmental meetings reviewing current literature and maintaining current licensure.
- Demonstrates initiative by contributing ideas to improve enhance or expand departmental services to better serve the residents of the facility.
Qualifications
- Required Masters Degree in social casework from an accredited graduate school of social work.
- LICMaster Social Worker (MSWMaster) STATEMI State of Michigan Upon Hire required
