Clinical Social Worker MSW Ambulatory Crisis provides consultation to staff and other health care providers. Serves as liaison between PIH Health and community to integrate services coordinate care for clients and enhance relationships through public relations activities. Assesses urgent or high-risk clinical situations and makes appropriate interventions. Assists in coordination with calling a Psychiatric Evaluation Team (PET). Assesses needs for mental health treatment and makes appropriate referrals as needed. Coordinates referral of patient to psychiatrist or other mental health providers. Conducts social work consultation and resource development. Performs case management coordination and monitoring of social work service plans in areas of personal social or economic resources conditions or problems.
The Clinical Social Worker MSW Ambulatory Crisis is responsible for additional duties as assigned by the Director including but not limited to clinical supervision for unlicensed social workers precepting MSW students and providing education and training to the healthcare team regarding social work services behavioral health palliative care hospice end-of-life care and regulatory requirements. Assists with administration development implementation and evaluation of social work programs and policies. Plans for and participates in development activities to keep abreast of current trends in medical social work. Participates in performance improvement. Implements Quality Improvement processes. Assists with development and implementation of departments annual work-plan. Performs clinical record audits. Maintains educational record and provides evidence of competency. Participates in departmental functions such as staff meetings staff in-services and committees as assigned. Participates in orientation of new personnel.

- Demonstrates initiative good judgment and the ability to make decisions independently
- Demonstrates skill in working with patients having planned and unplanned life transitions such as but not limited to diagnosis of chronic and acute illness death and dying and complex family dynamics.

- Knowledge with regulatory agency requirements legal policies and protocol
- Strong critical thinking and problem-solving skills to identify and resolve problems in a timely manner.
- Knowledge of managed care payment requirements and Medi-Cal and Medicare requirements.
- Demonstrates skill in working with chronic care and complex patients including assessing and managing psychosocial aspects of pain if assigned to Palliative Care.
