Job Description

Our biggest contributions to their independence are the clinical and nonclinical services we provide at our day centers.
The Program Director reporting to the Executive Director oversees critical PACE (Program of Allinclusive Care for the Elderly) functions that include but are not limited to nutrition social work day center personal care services scheduling and participant experience. In addition the Program Director is accountable for developing and maintaining a highfunctioning Interdisciplinary Team (IDT).
Essential Job Duties:
- Manage critical PACE functions including but not limited to hiring training supervising and mentoring staff and ensuring clear and effective workflows
- Develop plan and facilitate an effective IDT and other participantcare related meetings ensuring that they are organized and engaging and that followup items are documented and tracked
- Effectively oversee staff within their reporting structure to drive quality care and ensure adherence to Standard Operating Procedures (SOPs) and key regulatory requirements
- Collaborate to achieve high participant outcomes with shared participant services including but not limited to home care home health transportation central call center support and utilization management
- Oversee the WelbeHealth program site supplies and equipment management which includes monitoring and coordinating all ongoing maintenance and vendor oversight
Job Requirements:
- Bachelors degree in a relevant field such as health admin health care management nursing administration gerontology social work business administration; five (5) or more years of relevant management experience may be substituted; masters degree preferred
- Professional experience in a relevant field such as nursing social work psychology or recreational/occupational/physical therapy with demonstrated competence in working with frail elderly living at home required
- Minimum of seven (7) years of experience three (3) of which must be in a supervisory capacity managing a senior health care program outpatient clinic or healthcare facility
- Multidisciplinary management experience (nursing social work rehab)
- Valuebased care preferred

Salary/Wage base range for this role is $145399 $191926 / year Bonus Equity. WelbeHealth offers competitive total rewards package that includes 401k match healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
