Minimum Qualifications

MINIMUM QUALIFICATIONS:
Seven years of experience in providing health or developmentally disabled services counseling health care management or social services; OR

Six years of fulltime work experience plus four years of experience in providing health or developmentally disabled services counseling health care management or social services; OR

A bachelors degree plus four years of experience in providing health or developmentally disabled services counseling health care management or social services; OR

An advanced degree in a social sciences field nursing public health; public hospital or health administration; speechlanguage pathology or communication sciences and disorders; physical occupational recreational or music therapy plus three years of experience in providing health or developmentally disabled services counseling health care management or social services.

EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of fulltime work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

Job Specification

FUNCTION OF WORK:
To supervise professional and support level staff responsible for implementation coordination monitoring and evaluation of programs within offices specializing in services for those with developmental disabilities.
LEVEL OF WORK:
Supervisor.
SUPERVISION RECEIVED:
Broad review from a higherlevel professional or administrator.
SUPERVISION EXERCISED:
Directline over professional and supportlevel community services staff.
LOCATION OF WORK:
Department of Health and Hospitals (DHH) and Human Services Districts/Authorities statewide.
JOB DISTINCTIONS:
Differs from the Community Services Professional 3 by the presence of directline supervision of professional and support level staff.

Differs from the Community Services Manager by the absence of serving as the assistant to the regional administrator.

Job Duties and Other Information

Receive/screen inquiries/calls relative to persons contacting the Regional Office initiating appropriate disposition for assistance to person.

Conduct assessments refer and develop support profiles/support plans for persons seeking system entry or services within the Developmental Disabilities Service System. Assists individuals in choosing appropriate goals/services making recommendations for specific support needs based on information gathered by assessments and by personal interview and information from outside sources.

Analyze evaluative information identify and prioritize support needs/resources develop components of support and/or initiate referrals for support from alternative entities.

Outlines appropriate specialized services for program participants based on plan of support and other pertinent data gathered during interview with program applicant/participant.

Plans and coordinates statewide program elements as appropriate to need of individual.

Monitors applicable program elements for individual satisfaction and for fiscal soundness.

Create organize and maintain knowledge of community resources advocacy organizations and provider agencies based within assigned region/territory available for referral for supports for individuals.

Serve individuals and families in the primary parish within the region as well as any other parishes as assigned.

Maintain working knowledge of Developmental Disabilities state and federally funded resources.

Be familiar with Entry Process which includes a detailed review of medical psychological social and other evaluative information for presentation for review by entry review team consisting of credential professionals for entry decisions that require second level review of entry decision.

Prepare detailed plan outlining support needs and funding requests for presentation to family support review team in anticipation of funding decision.

Prepare appropriate documents for funding of approved agency services with thorough explanation to individual regarding fiscal process for reimbursement.

Attend court hearings and create supports and locate funding sources for individuals as ordered by a court representing agency and explaining agency supports to court personnel.

Respond to crisis situations as necessary to defuse the situation with agency resources or appropriate referral to outside agencies. Respond to Adult Protective Services referrals collaborating and accompanying APS staff as necessary to assist in reducing risk to individual in need.

Prepare reports maintain records and conduct professional correspondence relevant to all specialized areas/statewide program elements assigned.

Research and prepare appeal packet for supervisory approval for appeals within assigned programs submitting information to Bureau of Appeals within specified time frame.

Participate in appeal hearings before Administrative Law Judge presenting evidence for agency decision during hearing.

Serves as liaison and coordinator between individuals their families the community and service providers.

Point of Entry for the referral process to the Community Support Team (CST) for persons living in assigned region. Linkage to CST requires development of a detailed plan of support as well as gathering information from physicians psychologists etc. to accurately reflect the needs of the individual.

Assist IMCAL HSA in: special projects various IMCAL HSA activities Emergency Waiver Request updating Waiver Request information and other office functions as requested.

Networking meeting and serving as liaison with a variety of agencies and organizations (local state professional advocacy legal/court) to coordinate and maintain a service delivery system for individuals and families with multiple needs (such as participating on an ISC/interagency Service Coordination team representing IMCAL HSA at meeting).

Represent IMCAL HSA in a judicial trial for Family Support appeals in person or by phone to conduct fair hears and present the evidence of the agencys denial before the Administrative Law Judge the individuals and their families.

Be familiar with The Data Review Team (DRT) which occurs on a rotating basis to consist of at least 4 IMCAL HSA staff from the various programming areas to which they have knowledge (Early Steps Cash Subsidy Family Support Waiver Entry Unit Custody Certification ICF/DD PASAAR) facilitated by the Quality Enhancement Monitor at least quarterly.

Serves as a liaison at the 14th Judicial Mental Health Court as needed.

Refer individuals to Entry Unit Supervisor who have an expired Statement of Approval as needed for individuals on the Registry for Services Registry who are being offered a waiver slot or individuals requesting Family Support Services. Provide current information to Statistical Resources Inc. for all individuals who are being offered Waiver slots including those who have permanent eligibility. 10% Other duties:

Participate in staff and unit meetings as necessary and as requested by unit supervisor.

Participate in individual meetings with supervisor as necessary to keep him/her abreast of caseload situations.

Participate in interunit collaborations for successful coordination of services of individuals in the DD system of delivery.

Participate in professional and educational trainings to maintain and improve professional skills.

Participate as necessary in emergency preparedness training and activities to be prepared in the case of such situations.

Attend Emergency Preparedness trainings workshops and maintain readiness to assist with state identified emergency situations. Assist as needed with Special Needs Shelter preparation and operation.

Participate in agency internal committees as needed to assist the regional office in review of cases for eligibility and/or funding decisions.

Represent the agency as requested by supervisory personnel being prepared to explain agency programs as necessary to the general public and/or families/groups/service providers.

Participate and complete other duties necessary to office functions
