Job Description

To: All Teaching and Administrative Personnel Brockton Public Schools From: Dr. Kathleen F. Moran Assistant Superintendent of Human ResourcesRe: Nursing Case ManagerThe above captioned positions are brought to the attention of all teaching and administrative personnel. This position is a oneyear appointment and is pending funding. Qualifications:Registered Nurse in good standing with the Board of Registration in NursingLicensed with the Department of Elementary and Secondary EducationMasters degree preferredResponsibilities:Address the needs of students with chronic health conditions so students with these conditions may attend school in initiating a Bridge reentry program with adjustment counselors and other school staff for students returning to academics after extended absence for reasons such as post hospitalization rehabilitation or recovery or concussion to successfully return to in providing health education at the PreK12 level that addresses all aspects of using trauma and resilienceinformed in implementing additional projects such as responding to state and local emergencies pandemic response changes in state mandates and requirements for school health services an annual community health needs assessment that addresses health inequities and the impact of institutional racism and socioeconomic disparities on students and their academic achievement. Duties the Nursing Supervisor deems necessary and assist in all aspects of grant requirements. Compensation: Brockton Public Schools Contract 1.1 Admin 10 months 1 weekFunding:Comprehensive Health Services Grant
