Under the direction of the Chief Medical Officer the Director of Health Information and Population Health will engage in planning and implementing the use of the organizations data and information systems to support both distributed and centralized clinical operations and programs to achieve more cost effective and beneficial clinical outcomes for all patients. The Director of Health Information and Population Health will oversee other Clinical Directors who have oversight over specific populations at Salud (e.g. Pediatrics Womens Health Young Adult Health etc.). In addition the Director of Health Information and Population Health performs the duties of a medical provider.

DUTIES AND RESPONSIBILITIES:
DIRECT PATIENT CARE
1. Perform direct patient care; including history taking; physical examination; lab and radiology ordering and interpretation; prescription of medications; immunizations; and lifestyle modifications using established medical guidelines.
2. Perform procedures per documented training and clinical privileges.
3. Collaborate with supporting services at Salud to provide holistic care to patients including but not limited to Community Health Services Family Practice OB Behavioral Health Pediatrics Dental etc.
4. Refer secondary conditions to specialists when appropriate.
5. Institute emergency measures in lifethreatening situations.
6. Record all medical encounters in a timely and complete manner in the electronic health record.
7. Work with scheduling and management staff to ensure provider productivity standards.
8. Maintain valid medical license updated CPR certificate and valid CA drivers license.
LEADERSHIP
9. Assist the Chief Medical Officer in leadership duties as assigned.
10. Contribute to the monthly CMO report for the Board of Directors.
11. Facilitate and lead meetings related to population health including Patient Centered Medical Home.
12. Work closely with the Population Health Manager in a dyad partnership.
13. Support the CMO Chief Operating Officer (COO) and clinic managers with optimal patient flow.
14. Attend and participate in staff meetings including quality assurance quality improvement leadership meetings clinic administrative and management team activities.
PROFESSIONAL CONDUCT
15. Consistently adhere to and/or exceed SPLGs communication guidelines and expectations with internal and external customers including coworkers patients and external partners.

18. Facilitate the recruitment and onboarding of physicians/advanced practice clinicians.
19. May form liaisons with appropriate professional societies educational institutions and community organizations to enhance clinic ability to deliver medical care to target populations.
20. Adhere to and aid in the development of departmental policies procedures and objectives; ongoing quality improvement efforts and safety; environmental; and infection control standards.
21. Maintain patient confidentiality and comply with all federal and state patient confidentiality laws.
MANAGEMENT/SUPERVISION
22. Supervise the Directors of Population Health for various populations including Pediatrics Womens Health and Young Adult Health.
23. Work with clinic managers nurses and MA team leaders to ensure optimal patient flow.
24. May be asked to supervise students or residents or to coordinate placement with other providers.
CLINICAL INFORMATICS AND POPULATION HEALTH
25. Lead the development of Saluds population health goals prioritizing coordinating and evaluating the use of clinical data in collaboration with operational clinical and information technology leaders.
26. Inform and facilitate the operation of key information systems including electronic health records practice management and electronic dental records.
27. Lead use of data systems to ensure data driven decisionmaking.
28. Be responsible for the management of clinical data.
29. Ensure the quality and validity of data captured at all staff levels is consistent with operations and workflows.
30. Direct the development and facilitate reporting plans that support organizational needs and strategic objectives including: Mandated Federal and State reporting e.g. UDS HEDIS Meaningful Use the Central California Alliance for Health CareBased Incentive program grant applications and reports.
31. Participate and provide informed decisions regarding the adoption of new software and technology along with areas of improvement to streamline clinical operations improve quality and manage costs.
32. Direct clinical quality improvement efforts and participate in the Quality Improvement Committee monitoring performance of key indicators and leading improvement efforts as indicated.
33. Facilitate compliance with HIPAA and Saluds information security regulations.
34. Implement a population management program that helps drive decisionmaking.
35. Oversee panel management and teambased care to improve population management.
36. Engage with community partners and other resources within and outside the organization.
37. Collaborate with the Executive Team on key decisions and strategies.
38. Be a member of the Clinical Directors Team and Staff and Board QI Committees.
39. Provide and facilitate efficient and costeffective delivery of quality medical services to all patients.
40. Provide or arrange for inservice training to staff.
41. Use technology to improve health outcomes safety efficiency quality integration and care coordination.
MINIMUM QUALIFICATIONS:
Valid CA Drivers license CA Physicians License and DEA license
Current CPR certification
Board Certified or Board Eligible in their given specialty preferred
Masters in Public Health Clinical Informatics or related field or 2 years of relevant working experience.
Five (5) years experience working in a clinical setting at least some of that in an FQHC.
Fluent Spanish/English with the ability to treat counsel and educate primarily monolingual farm workers and their families in all health and patient care matters.
Experience with use of EHR systems Zoom Excel PowerPoint and Word.
PHYSICAL DEMANDS:
While performing the duties of this job the employee is regularly required to sit walk and stand; talk or hear both in person and by telephone; use hands repetitively to finger handle feel or operate standard office equipment; reach with hands and arms; and lift up to 25 pounds.
